HEALTH TESTS
The following Health tests are provided by the Oakland Health Department:
Grades 1-3-5 Vision Screening
Grades K-2-4 Hearing Screening
ILLNESS AND ACCIDENTS IN SCHOOL
Parents are notified immediately of an illness or injury and may be required to take the child home. If we are unable to reach you, we will leave messages at the contact numbers.
COMMUNICABLE DISEASES
Please telephone the school immediately if you child is diagnosed as having a contagious disease. The school must report this information to the Health Department.
MEDICINE
It is the responsibility of the parents and/or guardians to inform the principal and appropriate teachers if a student is subject to any medical need that requires regular or periodic attention while at school.
If prescription or non prescription medicine must be taken during school hours, the principal or his/her designee will supervise the taking of the medicine by the student. The student must bring all medication to the school office immediately when coming to school. NO MEDICATION MAY BE KEPT IN A CLASSROOM.
No medication of any kind shall be prescribed by or dispensed by school personnel without a signed permission/release form, from the parents or legal guardian, on file. Over the counter medications, such as Tylenol, cough drops, etc. will not be dispensed. Inhalants may be kept with the student as long as a doctor's note is on file in the office.
In circumstances when a child must take medication during school hours, he must have a written note from his/her parent or guardian AND a doctor's statement stating the following information which shall be placed in the child's cumulative file. Forms are available in the school office.
1. Name of medication to be dispensed.
2. Directions for administering medication.
3. Possible side effects which may be anticipated.
4. Length of time child is to receive such medication.
All medicine should be clearly labeled and the student's name must be on the container. Top priority will be given to the security of the medication.
TELEPHONE
Use of the telephone by students is limited to EMERGENCY calls only. Calls for permission to attend parties and ride buses with friends, etc. will not be permitted. These matters should be taken care of at home.
SMOKING
Smoking is prohibited anywhere in the school building or on the school grounds.

We are deeply concerned about the safety of your children. We ask that you abide by the following regulations:
Morning Drop-Off
TRANSPORTATION
Students who live in the Walled Lake School District have bus transportation provided in the afternoon by the district. Children from other districts must provide their own transportation. Written permission is required if alternate transportation is necessary. Permission to ride the bus must be in the form of a written note from both parties.
BUS TRANSPORTATION
Afternoon bus transportation is provided by the Walled Lake School District for those children living in that district.
